How to sign a declaration with a doctor in Kalush
A declaration is an agreement with a family doctor or pediatrician under Ukraine's state Medical Guarantees Program. Adults can sign it online from home; for a child, it is signed at the clinic.

Online
Since February 2026, adults can sign a declaration remotely through the electronic patient account.
- Log in to your electronic patient account — the state one or a private one.
- Find a doctor of our clinic in Kalush by surname: family doctor Ruslana Matsiashyk, pediatricians Oksana Obrizko and Olha Herasymchuk. Send a declaration request.
- Check your details and confirm the signing.

Signing at the clinic takes about 10 minutes. Please bring:

St. Panteleimon Family Clinic is a private family medicine clinic in Kalush (6/36 Bilasa i Danylyshyna St.). Two pediatricians and a family doctor, each with over 25 years of practice, see patients here; about 2000 patients have signed a declaration with them. On site: ultrasound, ECG, lab sample collection, vaccinations and pediatric rehabilitation massage. Open Mon–Fri 08:30–19:00, Sat 09:00–14:00.
Our goal is for every patient to receive quality medical care in an atmosphere of trust and compassion. Many families have been coming to our doctors for years: first the parents were patients, now they bring their own children.
The clinic works under the national declaration system with a family doctor or pediatrician, so the core check-ups and tests are free for patients with a declaration.

How do I book an appointment?
Call +38 (097) 699-53-84, send a request through the form on this website or message our Telegram bot. You can also book online via Health24 — the link is in the Booking section. After your request we will contact you to confirm the appointment time.
Do I need to sign a declaration?
A declaration entitles you to free services: doctor examination and follow-up, complete blood count (without differential), urinalysis, blood glucose, cholesterol and ECG. Adults can sign a declaration online through the electronic patient account; for a child, it is signed at the clinic.
Can I sign a declaration online?
Yes, if you are 18 or older. Since February 2026, adults can sign a declaration remotely through the electronic patient account, the state one or a private one: find our doctor by surname and send a request. A declaration for a child under 14 is signed by a parent at the clinic only, with the child's birth certificate.
What tests can I take at the clinic?
Among others: complete blood count and urinalysis, glucose, cholesterol, vitamin D3, immunoglobulin E, ferritin. Free for patients with a declaration: complete blood count (without differential), urinalysis, blood glucose and cholesterol.
Do you see children without a declaration?
Yes, we see all patients. Consultations without a declaration are paid separately — please call for the exact cost.
Are you open on Saturdays?
Yes. On Saturdays the clinic is open 09:00–14:00, on weekdays 08:30–19:00; Sunday is a day off. You can leave a request on the website or on Telegram at any time, and the administrator will call you back during opening hours.
What vaccines do you offer?
We have vaccines from the national immunization schedule (state-funded) and European vaccines. A doctor's examination is required before vaccination. Please call to check whether the vaccine you need is currently available.
